Example audit report. An audit report should include a number of points to ensure that the report is complete and effective. As an example, an audit report can have the following content:

Title page:

  • Name of the audit report.
  • Organisation and department to which the audit relates.
  • Date of the report.
  • Names of the auditors.

Table of contents:

  • A structured list of all sections and attachments.

Introduction:

  • Purpose and scope of the audit.
  • Background information on the audit topic.
  • Reason for the audit (e.g. legal requirements or internal quality control).

Audit Criteria and Standards:

  • Description of the norms, standards or benchmarks used for the assessment (e.g. ISO standards or internal policies).

Audit method:

  • Description of the audit methods and techniques used (e.g. interviews, document analyses, sampling).
  • Any limitations or challenges during the audit process.

Summary of findings:

  • A brief summary of the main findings and recommendations.
  • Note on the overall conclusion of the audit (positive, negative or mixed).

Detailed findings:

  • Specific findings broken down by section or topic.
  • Mention of strengths and good practices.
  • Identification of non-conformities or areas for improvement.

Recommendations:

An audit report is used to record the results of an audit, usually within organizations to evaluate processes, financial data, or systems. The report helps ensure compliance with legal and internal standards, identify areas for improvement, and provide recommendations. Sample audit report (digital method)

The basic structure of an audit report described is often the same. The method may differ. Usually the auditor establishes the finding(s), supplemented with metadata, and forwards the audit report to the auditee. The auditee determines the correction, does a root cause analysis, and then implements corrective actions to prevent recurrence. The auditor assesses the work of the auditee and finalizes everything.
